Title: Collisions and their Causes by Captain R. A. Cahill

Summary: Collisions and Their Causes is a practical maritime safety study in which Captain R. A. Cahill analyses real-world ship collisions to identify why they occur and how they can be prevented. Drawing from his experience as a master mariner, Cahill argues that most marine collisions are not caused by mechanical failure or poor visibility, but by human error. Captain Cahill’s central argument is that collisions are preventable. They result from predictable human mistakes rather than unavoidable circumstances. The book serves as both a warning and a training tool for mariners, emphasising discipline, rule adherence, alertness, and decisive action.

It remains a respected work in maritime training circles for its practical insights into navigational safety.
